Medical Records
Westgate Skin & Cancer maintains all medical records on behalf of patients per mandated requirements. Patients can access, retain, transfer, and make medical record requests below. Some services have associated processing and handling fees.
Medical Records
Medical Records are available for all established patients and can be accessed at no cost through the Electronic Medical Records (EMR) portal. For security purposes, registration is only possible from a welcome e-mail. Upon booking an appointment, all patients will receive a welcome e-mail to access the portal. If you cannot access the portal, please contact our offices.
Billing Records
Billing Records are available for all established patients and can be accessed at no cost through the Billing & Statement portal. For security purposes, registration is only possible from an e-mail statement. Upon the generation of your first bill, all patients will receive an e-mail statement. If you cannot access the portal, please contact our offices to review billing information.
Medical Record Transfers
Medical record transfer service requests can be submitted through an online form. This form, also called a release of medical records form, can be used to transfer records both to and from Westgate Skin & Cancer. Form submissions will need to be accurate and complete to avoid delays or denials in processing. A processing and handling fee, in accordance with state guidelines, will apply to all medical record transfer service requests. Medical records for established patients are accessible at no cost via the EMR portal as detailed above.
Dr. Epstein's Patients
When Dr. Anne Epstein retired in 2021, Westgate Skin & Cancer became the medical record custodian for all of her patients. Currently, all medical records are securely stored at our facilities and can be transferred to another facility upon completing a release of records form. Once requested, all paper records will be digitized and securely transmitted. A processing and handling fee, in accordance with state guidelines, will apply to all medical record transfer service requests. Dr. Epstein's records are not available via EMR since they are in paper format. Westgate Skin & Cancer will pay the processing fee if you transfer your care to our facility.
