Revelus Dermatology complies with state and federal standards when managing and maintaining patient protected health information (PHI). Patients can conveniently access, retain, and transfer their records at no cost through the self-service Electronic Medical Records (EMR) portal. All requests for the release of PHI have associated processing and handling fees. These fees are a reflection of the time and effort our staff dedicates to ensuring your records are accurately and securely transferred. We appreciate your understanding in this matter.

Protected Health Information (PHI) Request Specifics

A standard release, generally covering a few records, carries a flat fee. An extensive release, which demands additional time and effort, is calculated and priced according to federal and state regulations. A release request is required to determine fees.

Upon Dr. Anne Epstein's retirement in 2021, Revelus Dermatology assumed custodial medical record responsibilities for her patients. Should you transition your care to us, we'll cover the release fees to integrate all available records from Dr. Epstein's office into the EMR portal. Prior Dr. Epstein patients that have established care at our office are considered established and can access the EMR Portal. For all release requests, all available paper records will be digitized and / or duplicated as directed. All records provided “as is”.

  • Standard Release: $25.00 for up to 20 pages
  • Extensive Release: $25.00 for the first 20 pages, then $0.50 per page thereafter
  • Postage: If mailed, additional $5.00 per release

Should we refer you to another medical professional or facility, pertinent records for the consultation are typically sent at no charge. However, if extensive records or non-referred releases are requested, fees apply. All records are sent within 15 business days upon receiving an executed release and associated fees. For urgent cases, if the receiving facility informs us of an emergency, we aim to expedite the transfer, but fees still apply.

Non-medical third-party entities—such as insurance firms, attorneys, and trusts—are charged when requesting records on a patient's behalf. Please note that while patients can facilitate transfer requests, third parties can not directly access the EMR portal.

  • Standard Release: $25.00
  • Extensive Release: $25.00 for the first 20 pages, then $0.50 per page thereafter
